This also reflects my patient centered online learning portfolio A 35 year old female, resident of Nalgonda , farmer by occupation came to hospital on 19/9/22 with  CHIEF COMPLAINTS Headache and neck pain since 6 months . Headache was on and off and associated with vomiting. She complaints of hearing loss and tinnitus in right ear and giddiness since 1 month. HISTORY OF PRESENT ILLNESS        The patient was apparently asymptomatic 6months ago.The she had headache associated with neck pain and vomiting.  She complaints of hearing loss and tinnitus in right ear and giddiness while head movement since 1 month. C/O nausea and vomiting since 3months C/O no fever HISTORY OF PRESENT ILLNESS  Patient is apparently asymptomatic 3 years ago. Then she developed pain in the right groin region which radiated to right flank region It was gradually increasing pain. This also reflects my patient centered online learning portfolio   40 years old male  resident of nalgonda who is farmer came to the hospital with the complaints of  CHIEF COMPLAINTS C/o  Fever since 3 days  C/o cough (dry cough) since 3days  C/o shortness of breath grade 4 since 3 hours  HISTORY OF PRESENTING ILLNESS Patient  was apparently asymptomatic  3days ago then he had fever  associated with chills and rigors  then he developed dry cough,And then developed SOB Grade 4 since 3 hours. This also reflects my patient centered online learning portfolio This is a case of 38 year old male came with cheif complaints of  1.Fever with chills 2.cold 3.Cough 4.Generalized weakness 5.Head ache(frontal region) 6.SOB grade 2 7. Loose stools HOPI:- Patient was apparently asymptomatic 4days back. Then he developed fever which was high grade fever with chills which eas intermittent relieved by medication associated with cold,cough which is dry in nature and generalized weakness. Since 1 day he has complaints og loose stools of 5 episodes which is watery and non blood stained and non mucoid.No complaints of pian abdomen or vomitings. PAST HISTORY:- Not a known case of HTN/DM, /Thyroid disorder/TB/Asthma/CAD PERSONAL  HISTORY:- DIET- Mixed APPETITE- Normal SLEEP- Adequate  BOWEL AND BLADDER MOVEMENTS - regular  ADDICTIONS -none
